Application for Upgrade
Referees requesing an upgrade must complete the Application for Upgrade and return the form to the State Director of Assessment (SDA), 1153 Owens Road, Auburn, AL 36830. The educational requirement and physical exam (see requirements for Grade 7, 6, and 5) must be completed before the field evaluation(s). A minimum of two-weeks notice prior to the match is needed to assign an assessor. The fee for upgrade must be remitted with the application: Grade 8 Upgrade,
$60 and Grade 6 & 5 Upgrade,
$125. Referee, Grade 7, State Referee, Grade 6 & 5, maintenance assessment fee is
$60 and must be sent to the SDA (address above) prior to the assessment request. Application for Upgrade

Report of Assessment of Game Officials
The Word document form can be filled in and printed. The form is to be completed by the Assessor and sent to the SDA (address above) within 7-days of the match.
