U13 & Up $375 per team plus $200 bond--

ITEMS THAT NEED TO BE PRESENTED AT CHECKIN:
- An official 2010-2011 State Approved AYSA roster- will be provided to you at check in by AYSA
- Valid 2010-2011 laminated player passes (which include signature and photo of player printed on pass or attached)
- Valid 2010-2011 coaches passes also laminated with a picture and signature for each rostered coach (ONLY FOUR CARDED ADULTS ARE ALLOWED ON THE SIDELINES WHETHER IT IS MANAGERS/COACHES)
- Medical Release form for each player on roster - DOES NOT HAVE TO BE NOTARIZED
